Description

The Haven is a low-barrier day shelter and housing resource center for those experiencing homelessness and extreme poverty in the greater Charlottesville area. As a part of The Haven’s Housing team, the Veterans Case Manager serves local veterans by helping them find permanent housing and become stabilized in that housing–this includes veterans who are literally homeless or imminently at-risk of losing their housing. The Veterans Case Manager also connects veterans to other community partners and services, helping them navigate the housing search and leasing up process. The program’s progress is reported both to The Haven and to the primary contractor (Virginia Supportive Housing).

Job Duties

  • Developing individualized case plans for each veteran client which identify the veteran’s goals and what action steps should be taken to achieve those goals.
  • Assisting veterans in obtaining employment or higher-paying employment.
  • Assisting veterans in applying for any benefits for which they may be eligible, including V.A. pension benefits, social security benefits, etc.
  • Working with local landlords and property managers to identify suitable housing for local veterans and to serve as a liaison between veteran tenants and their landlords.
  • Conducting outreach to local homeless veterans, to community partners serving the homeless community in general and homeless veterans in particular, and to local landlords.
  • Maintaining an accurate accounting of all program funds spent and reporting on the status of the program’s funding on a monthly basis.
  • Creating, updating and maintaining an accurate case file for each veteran, in accordance with the primary contractor’s guidelines and with the Veteran’s Administration’s program guide.
  • Entering client information into a secure database and updating and maintaining the data with the database.

Skills Required

  • Familiarity with the Motivational Interviewing and Critical Time Intervention methods of case management.
  • Being detail-oriented and able to maintain and submit significant paperwork for each client.
  • Excellent oral and written communication skills and computer skills to be able to maintain an online database and accounting spreadsheets.
  • Being a self-starter, capable of completing tasks within an assigned time frame.
  • Good interpersonal skills with a strong sense of humor.

Work Schedule

Monday – Friday / 9:00am – 5:00pm
