PACEM Case Managers work full-time throughout our shelter season (currently late October through mid-April), and less during the summer months. Primary responsibility is to provide intensive community based social work services including coordination, advocacy, support and encouragement. Intensive work with current guests to remove barriers to housing and create plans to transition guests to stable housing as quickly as possible. Work will facilitate access to mainstream community resources, especially in the areas of substance abuse, mental illness, aging, etc.

Job Duties

The Seniors Case Manager focuses on our oldest clients (50+ years of age) in the shelter, and provides ongoing case management for individuals housed through our Secure Seniors program.

Key Responsibilities:
• Serve as advocate for senior (50+ y.o.) shelter guests
• Serve as counselor for housed clients of the Secure Seniors Program
• Follow established case management policy and practice
• Identify and prioritize improvements to case management policy and practice (i.e., contribute to continuous improvement)
• Collaborate with other case managers on staff
• Maintain shared client files in multiple formats (paper, digital, and online)
• Supply reports to Board and Executive Director at regular intervals and as needed
• Support shelter operations and shelter directors
• Build partnerships and the ability to refer with an array of community agencies

Skills Required

Preferred Qualifications:
• Bachelor’s degree or equivalent in social work or related field; seniors focus a significant plus
• 2+ years experience providing direct services to individuals experiencing homelessness, poverty, addiction and/or mental illness; seniors focus a plus
• Effective oral and written communication
• Strong organizational skills with ability to work independently
• Strong collaboration skills with ability to prioritize multiple and varied tasks
• Cultural competency, including sensitivity to seniors’ issues and ability to relate to people of diverse lifestyles and cultures
• Knowledge of community resources for seniors including supports for: mental health, physical health, substance abuse recovery, neglect and/or abuse, and other services
• Knowledge of community resources for individuals experiencing or at risk of homelessness, including mainstream social services

Work Schedule

Full-time, year-round position.
Monday through Friday, 9am-5pm; flexes during shelter season to accommodate two evenings (6-8pm) per week.
Roughly 60% in office, 40% in field (varies across the year and with caseload). Reliable transportation required.
Anticipated start date for new hire is August 1, 2024.
