Description

JABA is seeking someone who is enthusiastic about helping older residents and adults with disabilities to identify resources and to coordinate services to support living in the place of their choosing. The position is based in Louisa and requires significant local travel. Candidate must have ability to work with a team of JABA staff and other community partners, to develop person-centered relationships and to serve as an advocate for individuals as well as in the community.    Requires knowledge of local and state community services, human services resources, aging and disability issues, casework principles, and person centered planning. Proven ability to use computer to access internet, use applications including MS Office Word, Excel and Outlook, and online data management systems to document services.  Must be able to lift 10 lbs., climb stairs, walk, stand, stoop/bend, and perform repetitive hand motions.

Skills Required

Bachelor’s degree preferred and or extensive case management experience in the field of human services, preferably in aging or disability services. Alliance of Information & Referral Systems (AIRS) certification within one year of hire; training for this certification must begin with six months of hire.

Work Schedule

Hours are 37.5 per week. Normal schedule is Monday – Friday, 8:30am – 5:00pm.
