Overview:
The Housing Stabilization Case Manager I supports newly-housed clients to gain stability through connections to community resources and mainstream benefits.

Job Duties

Basic Responsibilities:

  • Conduct program enrollment meetings, including Rapid Re-Housing
  • Assist Client with initial move-in needs in coordination with other program coordinators
  • Work with Clients to create housing stability plans, moving them towards self-sustainability
  • Establish transparent, collaborative, and trusting relationships with Clients to promote and support housing stability
  • Advocate on behalf of Clients for eligible services and housing opportunities, such as Permanent Supportive Housing
  • Refer Client to other community resources when needed and follow up to ensure referrals were completed
  • Attend assigned community-wide meetings to discuss and troubleshoot issues regarding housing stability
  • Conduct ongoing outreach to establish a network of community resources and services
  • Become SOAR-trained (after being hired)
  • Complete quality assurance surveys with Clients during and after completion of the program
  • Maintain digital files and notes in HMIS database
  • Contribute to the development and refinement of housing program policies

Skills Required

Skills Required:

  • Experience in housing program case management or direct care with vulnerable populations
  • Experience working with populations challenged with homelessness
  • Social justice-oriented, a team player, and committed to working alongside families as they transition out of homelessness
  • Driver’s license and regular access to reliable transportation

Additional Info

Desired Skills:
  • Familiarity with: Motivational Interviewing, Critical Time Intervention, Progressive Engagement, trauma-informed care, harm reduction
  • Excellent oral/written communication skills in conjunction with computer/data entry skills for maintaining online database and other records
  • Being a self-starter, capable of completing tasks within an assigned time frame
  • Good interpersonal skills and good sense of humor; team player
  • Ability to think outside the box and adapt to dynamic work settings
  • Good personal/professional boundaries and self-care practices
