Job Description: Housing Program Coordinator – Homeless Prevention; Rapid Re-Housing; Coordinated Access
Reports to: Executive Director
Takes direction from: Community Case Review, colleagues, clients
Basic Responsibilities:
Housing Program Coordination:
- Provide oversight for entire Housing Program staff through:
- regular supervision meetings
- facilitation of Housing Program meetings on the 2nd and 4th Thursdays of the month
- fiscal planning and oversight of Prevention and Rapid Re-Housing budgets
- revise Housing Program Manual (as needed)
Prevention Administration:
- Work together with Intake Coordinator to determine eligibility and funding allocation
- Using written guidelines from Housing Program Manual, administer state prevention funds on behalf of the continuum of care
- Work with client family and/or friends, to divert clients from entering (or reentering) the homeless system of care
- Work together with Housing Navigator to engage with landlords, rental agencies
Rapid Re-Housing (RRH) Administration:
- Work together with Intake Coordinator to determine eligibility and funding allocation
- Using written guidelines from Housing Program Manual, administer state RRH funds on behalf of the continuum of care
- Work together with Housing Stabilization Case Managers to create plans for stability
- Work together with Housing Navigator to engage landlords, rental agencies
Coordinated Access:
- Together with the Intake Coordinator, develop, implement, administer, and maintain a low-barrier coordinated access packet (CAP) to ensure that all clients gain access to area resources, including the Community Case Review agenda, and are oriented to the rules and expectations of The Haven
- Gather and assess information about clients including basic demographic data, simple history and their medical, housing, and social services needs
- Work together with Intake Coordinator to enter CAPs into the Homeless Management Information System (HMIS) on a weekly basis
- Support guests in getting connected to area services through information & referral and by addressing barriers to maintaining or securing stable housing
- Maintain a working knowledge of area service provider eligibility requirements
- Develop appropriate working relationships with area service providers to ensure that guests have adequate access to services
- Contribute to the development and refinement of procedures and protocols necessary to operating The Haven
Skills Required
- Experience with administration of state-funded housing programs (e.g., Rapid Re-Housing and Prevention)
- Experience managing personnel
- Experience managing a program budget
- Detail-oriented
- Works well within a team setting
- Ability to work in a fast-paced and interactive work context
- Good interpersonal skills
- Highly motivated and organized
- Good-humored
Work Schedule
9:00am to 5:00pm; Monday – Friday
