Care Transitions Coach

Full-time position’ to be part of exciting Care Transitions Program. Coach will work directly with patients and caregivers in their homes, using a nationally recognized model, to assist with a safe transition from the hospital.  The goal of the program is to prevent unnecessary re-admissions to the hospital by increasing patient ability to manage their own healthcare. Specific training for new coaches is provided. Position requires excellent communication skills, and ability to work with senior citizens in their homes. Candidates will need flexibility, written and computer documentation skills. Position requires significant amount of driving with the PD 10 area, basic computer literacy, high degree of reliability and attention to detail. Position also requires strong time management skills, ability to work at high energy level.   High school diploma required, post-high school coursework, or related job experience preferred.
