Description

JABA is seeking qualified Care Coordinator, who is people orientated and compassionate, and will assist older adults and persons with disabilities with services to live independently.  Duties include assessing individual needs, developing plan of care, arranging home or facility visits, documenting information and complying with HIPAA. The position requires frequent travel to home and facilities in the City of Charlottesville and surrounding counties. Must possess excellent verbal and written communications skills and intermediate computer skills in Microsoft Office and web based software programs.

 

 

 

Skills Required

Bachelor’s Degree in social work or related field with two plus years’ experience in community based services.
