Care Transition Coach

Exciting opportunity to work with JABA in their new partnership with area Managed Care Organizations. We are seeking a full time Care Transitions Coach who will work directly with patients and families to ensure effective transitions from hospitals to home. Our program includes the use of evidence based practices, designed to develop patient self-advocacy skills and reduce preventable readmissions to hospitals. Duties include but are not limited to meeting with patients in their homes to assist them with the development of a personal health record; practice medication management, schedule follow up appointments and enhance patient doctor communications.

Position also includes communication with other health care team members and making necessary referrals to other JABA programs.  Must possess excellent listening skills along with strong ability to communicate both verbally in writing.  Excellent organization and ability to work independently is required for this position. Proficient computer skills, including data entry. Position requires a significant amount of driving throughout Charlottesville and surrounding counties.
