Options Counselor

JABA (Jefferson Area Board for Aging) is seeking an Options Counselor for older adults and individuals with disabilities. The Options Counselor supports clients in their deliberations about remaining in community-based living and/or other long-term support choices. Duties include: Utilizing person centered planning to maximize the person’s participation in their long term care planning process, coordinating services to implement the service plan, and monitoring the delivery of services. Requires knowledge of aging issues, casework principles, person centered planning and program design and management. A college degree is preferred and/or extensive case management experience in human services. Must have demonstrated ability to communicate effectively orally and in writing, work independently and as a team member, and build excellent working relationships. Requires a valid driver’s license, reliable transportation and skill in basic computer applications including Microsoft Office. Must be able to lift 10 lbs., climb stairs, walk, stand, stoop/bend, and perform repetitive hand motions. This is a full-time position to provide options counseling in Charlottesville and Fluvanna, working 37.5 hours per week. Normal schedule is Monday – Friday, 8:30am – 5:00pm.
